I am not saying that paramedics are the be all and end all of prehospital care - just that it is the one area that we practice in and so develop a very specific set of skills and knowledge relating to it. I agree that there are many who believe that? they are somehow "better" than a doctor or a nurse, and I understand your wanting to clarify that. In my experience on the road, the majority of other paramedics I have worked with understand only too well that healthcare is a team effort. And yes, we do operate under standing orders from a doctor, but doctors also have to answer to someone...they are not wholly autonomous and self-regulating. Thank you for an intelligent debate.

Lastly, I did not imply that paramedics are one step above? a stretcher bearer. I was pointing out the irony of the statement 'doctors playing in pre-hospital care.' Without standing orders from a doctor, a paramedic would have no authority to practice any skills what-so-ever, so as valuable a contribution as a paramedic may and generally does make, it would be a gross overstatement to suggest that paramedics have any more 'right' to practice in pre-hospital care than doctors do.

It is a pathetic point of view for a paramedic to take because health is team work but paramedics are the first to forget that because they generally work separate to all of the other HCPs, doctors, nurses, physios, speech path, dietitians, psyche, etc. They forget that if it wasn't for everyone else, just like if it wasn't for paramedics, the system would be broken and would help? nobody. I appreciate your concern. I trained as a paramedic before starting as a doctor and I understand that all links in the chain of survival were important. The implication of my post was not to paint paramedics with a large brush but rather the self righteous type that believes they have more to offer than doctors because they were trained how to jam a tube down someone's throat. As much as I appreciate your reply is primarily aimed towards one specific person - please do not lump all paramedics in as "uneducated hicks". We have extensive training and knowledge in specific areas (i.e. prehospital care) and it is accepted by a large majority of doctors that we do a demanding job very well...a lot of doctors have confessed that they would be unable to do our job. In the UK CURRENTLY - only a qualified doctor is allowed to perform RSI. Changes are currently going through that would enable paramedics to possess the drugs to carry out RSI, but NOT YET. If it does happen, it will be CCP's (critical care paramedics). ECP's (emergency care practitioners) work with minor injuries and primary care centres, as well as normal paramedic day to day? work, not critical care or trauma (at least, as an ECP).

I am almost 110% sure the ECPs in the UK... and you only get ECPs in UK and S.A, can also do RSI-Rapid Sequence Induction- (What the Doc did on scene). That Doc really sounded South African, wouldn't be surprised, with the high demand for all our trauma medical staff around the world due to the quantity of trauma calls we see. As a former (i.e. "burned out") "advanced scope of practice paramedic" in California I was allowed (as wll as "non advanced" EMT-P) to? perform RSI with consultation with the medical director. Also, I was a little surprised to see that the lung injured patients didn't get a needle decompression or chest tube prior to transport... For our air ambulance folks this is a normal practice especially when transporting a patient in an air ambulance. But, short transport times... Maybe less time on scene?

I had to open your message just to read what another typical yank medic with a god complex had to say about doctors in prehospital care. Fact of the matter is that these doctors 'playing' in prehospital care know more? about the human body and how to fix it when it goes wrong than paramedics. That's why they are called doctors. RSI which was performed in this video could easily become a Paramedic skill. After all Paramedics can intubate .CCP's performed around 49 last year, none required any further interventions by the Doctor on scene. Just a case of Doctors? dictating pre hospital care practice.If you read the SecAMB CCP it is reported that paramedics under some circumstances have fewer deaths due to trauma than Doctors. Hospital is the place for seriously ill people.

A 4 minute response from the royal London hospital in North London too south London is not at all bad!!! If they have to land 1/2 a mile way then normally there will be a police car there to pick them up! The problem ambulance crews have in london is that there are different hospitals that specialise in different things... If you have a patient in need of a major trauma centre and it's rush hour then good luck trying to get there anytime soon by road!!? I've worked 2 jobs with HEMS and they are

HEMS Saves lives. So what if it takes longer to get them to hospial, if something happens on the road to hospital and the Paramedics do not have the knowledge, authority, or tools to fix it, well. I would and do feel a lot safer knowing that a doctor is comming to look at me on the scene to assess my movement and transportation. Englane operates differently, and the success rate of HEMS is high, they provide amazing and needed care where it is critical. A mate is in the London Ambulance and what you say is not true. Ambulance crews have to run observations and make sure the patient is stable before transport. 20 mins minimum. 2 Man crew with one driving means you don't want a patient needing resus en-route so they stabilise prior to transport. In addition, paramedics take years to? train and there's just not enough. Most crews are Technicians so can't administer pain relief.

my point is that by the time it takes a helicopter crew to take off, respond to the area of the incident and then land half a mile or more from the scene forcing their crew to run? with all their gear through urban neighborhoods, an ambulance crew could have the patient packaged and already at or near a hospital. It just seems like to me that ambulances are waiting around just so the HEMS crew can assess the patient when the ambulance could already be at the hospital if they didn't wait.

Although the type of system is important and does have an effect on patient care, it is nevertheless totally independent of protocols and scope of practice. You are right though, scope of practice won't be curtailed on nationally. It just? doesn't work that way. My point was that the way it's done in the US does not always = the best way. It may still be good to have a doc on scene even if he only does things that a US medic is allowed do. Higher RSI success rate for example.

A quick glance at the NREMT site shows that 46 states? require NREMT certification either initially or for renewal. That doesn't support the claim that EMS practices vary widely across the US because in that case a standardized curriculum wouldn't be applicable. Why require people to be trained to a standard that's totally different from what they will do? It does support the claim that the majority of EMS standards in the US are minor variations on the same theme.

Also, the delivery of EMS also varies greatly from city to city. In one city you might have an All ALS system filled with cross trained Firefighter/Paramedics. In another city you might see a third service system where the EMTs and Paramedics have their own agency seperate from the FD and PD. In yet another system you might see a private ambulance service provide EMS for an area. These differences are huge. Protocols for? each area can vary immensely as well.

That's misleading. Although there are minor differences, the majority of the EMS systems in the US stick very closely to the NREMT/DOT model. But your post speaks to one of the bigger problems in US EMS- the paramedic god complex. Paramedics often fail to understand that what they are taught is a tiny slice of medicine, pared down and simplified, to train them for a specific job in as little time as possible. It is better? to have a doc on scene simply because they know more.

As far as surgery to stop internal bleeding, that's precisely what a thoracotomy is so yes in some cases they can. The reason scene times are drilled into paramedics is because they are not in a position to decide.

US has so many different EMS systems that not only change state by state but city by city. I don't see the scope of practice for paramedics being curtailed on a nationwide basis because no one place does it the same way.

The reason for the rush to the ER in the States is that paramedics can't deliver definitive care. If there's a doctor on scene with sufficient equipment, they can start definitive care in the field and scene time becomes a lot less important. Also, take a look at the studies regarding RSI (or ordinary intubation for that matter) in the US. Paramedics have a much lower? success rate than doctors. Don't be surprised if the scope of practice of US medics is curtailed fairly soon.

Here's a wakeup call: not everyone does it the same as you do! And in urban environments, you can easily breach the 30 minute distance from hospital (look up travel times from the edge of the M25? to central London, for example. Look into paramedic outcomes on complex procedures such as RSIs (failure rate of 1.3% for HEMS doctors compared to 7.3% for US ground paras and 20% for nurse aeromed teams (!!!) ). How often do you perform an RSI compared to dedicated HEMS anaesthetists?

I'm not trying to knock the way things are done in the UK. I'm just trying to highlight the differences. We don't use a helicopter unless we are 30-60 minutes from the closest? trauma center or if there is a lengthy extrication involved. Even so, helicopters are over-utilized in the US. Doctors don't fly on the helicopters in the US. You have Flight Paramedics and Flight Nurses.

I am medically trained. I'm a Paramedic in the United States. We don't need a doctor on the scene to do this stuff. I can do this in the field on standing orders. I've dealt with bi-lateral femur fractures. We have traction splints that we use to straighten the femurs and we have pain medication? and procedural sedation on standing orders. We can also do RSI after calling medical control. Needle decompression, yes. Chest tubes and thoracotomies no.

They do an amazing job,? people just don't realise how many people are saved by the skill of the doctors, paramedics and nurses. The helicopter is great and it is good to see the emergency services helping each other out. Where I live, the police will not even protect the fire service crews when they are being attacked by yobs. A lot of work goes unnoticed and it's only until you have an accident or see one that you see what goes on and how many people are involved. Good TV show!

Im fully qualified medic and i can say that a big problem is that our emercency doctors have only an 40 hour cours after their study! So every gynecologist and GP can do that course and work as a emercency physican and thats far to little education.So we have good qualified medics and often bad doctors. I think good docs are important for? a professionell ambulance service, but quality is better then quantity. Switzerland is showing us how it would work with a combination of doctors and medics.

Our Ambulance service is? based on emercency doctors. In Vienna we have 1 Helicopter vor 2mio people. Whole in whole we have 14 doctors in Vienna, normaly on rapid response cars The paramedics have different education levels. With the highest level yo are also allowed to perform i.v , intubation and applicate some drugs. Unfortunately Vienna is the only area where we have a professional paramedics, the rest is volunteer based. So medics in rural areas have normaly low education levels.

Well said mate. As one who has paramedic and nursing degrees and is now in medschool, I can tell you that medics have a lot of holes in their knowledge. There is a reason that doctors are awarded the title 'doctor' and paramedics are not. No amount of jealousy and dick measuring will change that. The HEMS doctors are not 'just' doctors? but trauma specialists. Keep in mind that each paramedic is only using their skill set and illusion of autonomy through guidelines written by doctors.

@Emsworker68. Most of the DRs on HEMS have been or are surgeons / A+E(ER) consultants and have been known to do surgery in the street such as? amputations and even in some cases opening the pt's chest, things that a CCP can only dream of doing. Hope this helps

It has nothing to do with the fact one has a medical degree or not. I live in Canada and? in my province we have critical care paramedics. I just don't see what a doctor in a field can do that a CCP cannot do. I guess it comes down to standing orders. If you give them the training and the tools to the job there would be no need for MD's in the field. I still think it is a waste for a doctor to have them go on calls. Some US states use RN's also. That too is odd!

Im not sure about the UK but? where I come from paramedics dont have to have a medical degree. Therefore a doctor has a much highter level of training. Also a doctor is allowed to give medication that a paramedic is not allowed to touch.

I agree. I find taping it with the needle in can? blow the site. I like making sure it is patent also. I do not understand why they need a doctor on the helicopter. She could not do any more than a critical care paramedic could be made to do. Seems rather a waste of resources.
